Une fois le formulaire rempli, cliquez sur le bouton vert « Envoyer » et puis vous allez recevoir dans votre boite mail votre exemplaire signé.
Il est nécessaire par la suite de nous transférer ce mail à partir de votre boite mail à l’adresse mail suivant :

En indiquant impérativement la mention suivante dans la première ligne de votre mail :
« Document lu et approuvé »
N’oubliez pas d’inclure le document signé en pièce jointe à votre mail transféré !
Nous vous enverrons par la suite une confirmation de bonne reception.
